Direct Support Professional

Reports To: Team Leader

Program Supervisor: Executive Director

JOB SUMMARY

Full-time and Part-time hourly non-exempt position. Chief responsibility is to ensure the health and safety of each individual resident, ensuring you interact respectfully with the resident, by supporting and encouraging him / her to make choices, develop independence, and to participate in all areas of his / her community life. This is primarily accomplished through direct support and ensuring compliance with all COC policies & procedures and applicable State OAR’s as well as the implementation of each resident’s Individual Support Plan (ISP).

AGENCY FUNCTIONS

  • Promote COC’s Mission and Values
  • Uphold COC’s Policies and Procedures
  • Attend and participate in COC’s Team Meetings, as scheduled

MINIMUM QUALIFICATIONS

  • Be 18 years of age or older
  • Pass Criminal History Background Check
  • Pass pre-employment drug screening
  • Possess a High School Diploma or GED
  • Possess a valid Oregon Driver License
  • Be insurable by COC’s auto insurance carrier to drive agency vehicles
  • Have a negative TB test result

  • Lead by example, participate, coordinate and cooperate with your team members and all other COC employees and community partners
  • Exercise sound judgement
  • Detail oriented and self-directed
  • Strong public relation skills, ability to work with families, general public, and people with disabilities
  • Must be able to follow directions, both written and verbal
  • Must be able to attend all assigned shifts, unless advance arrangements have been made

PHYSICAL DEMANDS

  • Must be able to lift a minimum of 50 lbs., assisted or unassisted, to transfer or lift products or non-ambulatory residents
  • Must be able to listen and communicate effectively
  • Must have the ability to meet the following physical demands:

TRAINING AND CERTIFICATION

  • Certification in CPR and First Aid
  • Certification in Oregon Intervention System (OIS)
  • Complete Core Competencies with 60 days of hire
  • All other trainings as required by COC policies and administration

PHYSICAL DEMANDS

  • Must be able to communicate and listen effectively
  • Must be able to lift a minimum of 50 lbs.
  • Must be physically able to transfer a person between two points with assistance, which may include twisting, turning and / or bending while holding an individual

MAJOR RESPONSIBILITIES

  • Provide supervision, training, and support to all residents at your work site and others as needed
  • Be intimately familiar with each resident’s ISP, contributing in its development, and endeavor daily to ensure that the ISP goals and action plans (resident’s current needs and personal goals) are pursued and completed on time and effectively monitored

OTHER RESPONSIBILITIES INCLUDE (but not limited to)

  • Hands on involvement with resident’s daily routine and needs
  • Assistance for residents to perform their personal care, including hygiene, grooming, exercise, range of motion, bladder and bowel care
  • Dispensing of medications and administering medically prescribed treatments
  • Maintaining a clean and attractive home
  • Working cooperatively and effectively with Community Inclusion team leader and staff
  • Preparing nutritious meals according to dietary requirements
  • Compliance with house etiquette and rules, MAR books, medication dispensing and documentation requirements
  • Review of pertinent medical history, Behavior Support Plans (BSP), protocols and individual profiles
  • Location of cleaning supplies and equipment, food supply, safety equipment, utility systems, and emergency preparedness
  • Awareness of all fire drill requirements
  • Taking residents to medical and dental appointments as request by team leader
  • Maintain all records of house and vehicle safety and safety and maintenance
  • Working with the Client Services Director, ensure the accuracy of resident fund oversight and that residents have adequate access to their funds
  • Provide transportation utilizing company vehicles and supervision to ensure residents consistently access and engage in preferred community activities
  • Perform other tasks and assume additional responsibilities as assigned
  • Attend and participate in a minimum of 4 hours of pre-service training and 12 hours of job-related in-service training annually thereafter
  • Perform other tasks as assigned by Team Leader, Quality and Operations Manager, or the Executive Director
  • Turn monthly documentation in to by the first of each month
